ACCESS Model: Understanding CMS’ Model to Expand Access to Tech-Supported Care
The Centers for Medicare & Medicaid Services (CMS) recently proposed a new voluntary model – Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model – which tests using outcome-aligned payments to expand access to technology-supported care for people with chronic diseases. The ten-year model begins July 5, 2026.
